SmarterMail Administrative Authentication Bypass (CVE-2026-23760)

Historical catalog analysis: CISA added this entry on January 26, 2026. The entry reflects catalog information retrieved on August 31, 2026, not a snapshot archived on the inclusion date. Check current vendor guidance before acting.

What the vulnerability is

CVE-2026-23760 is an authentication bypass vulnerability (CWE-288) affecting SmarterTools SmarterMail. The flaw exists within the password reset API, specifically at the force-reset-password endpoint. This endpoint allows anonymous requests and fails to require a reset token or the existing password when targeting system administrator accounts.

An unauthenticated attacker could leverage this path to provide a target administrator’s username and a new password, effectively resetting the account credentials. This results in full administrative compromise of the SmarterMail instance. CISA has noted that this vulnerability is known to be used in ransomware campaigns.

Exposure and applicability

This vulnerability is highly relevant to U.S. healthcare providers, practices, and business associates who utilize SmarterMail for organizational communication. Because email servers often handle electronic Protected Health Information (ePHI) and coordinate critical clinical operations, an administrative takeover of the mail server could lead to unauthorized access to sensitive patient data or the deployment of ransomware across the network.

Organizations should determine if they are running a version of SmarterMail that is susceptible to this bypass. While CISA has set a remediation deadline of February 16, 2026, for federal agencies, healthcare entities should treat this as a high-priority risk due to the confirmed exploitation in ransomware attacks.

Remediation priorities

Based on our analysis, we recommend the following prioritized actions to reduce the likelihood of compromise:

  1. Immediate Patching or Mitigation: The primary priority is to apply vendor-supplied mitigations or updates as specified in the SmarterTools release notes. This addresses the root cause by securing the password reset API endpoint.
  2. Responsible Role: IT Administrator / System Engineer.
  3. Administrative Account Audit: Review all system administrator accounts for unauthorized changes or new, unrecognized admin users that may have been created via this bypass.
  4. Responsible Role: Security Officer / HIPAA Compliance Officer.
  5. Network Access Restriction: If immediate patching is not possible, restrict access to the SmarterMail administrative interface to trusted internal IP addresses only, reducing the exposure of the vulnerable endpoint to the public internet.
  6. Responsible Role: Network Engineer.
  7. Decommissioning Evaluation: For organizations unable to apply mitigations or those with outdated support contracts, evaluate the discontinuation of the product in favor of a supported, secure alternative.
  8. Responsible Role: IT Director / CIO.

How to validate remediation

To verify that exposure has been reduced, administrators should coordinate with their security teams to perform the following:

  • Version Verification: Confirm that the installed version of SmarterMail matches the patched version specified in the vendor’s official release notes.
  • Configuration Review: Verify that network-level access controls (such as ACLs or Firewall rules) are actively blocking anonymous external requests to the administrative API endpoints if patching is delayed.

Note: Version checks alone do not guarantee total security, but they provide evidence that the known vulnerability has been addressed.

Limits and open questions

Applying vendor mitigations could reduce the likelihood of this specific authentication bypass; however, it does not eliminate all ransomware risks or other potential vulnerabilities within the mail server.

It remains unknown exactly how many healthcare organizations have already been targeted by the ransomware campaigns mentioned by CISA. Additionally, while the entry path (the force-reset-password endpoint) is identified, the full extent of post-compromise activity an attacker might perform once administrative access is gained depends on the specific configuration and integration of the SmarterMail instance within the broader healthcare network.
